Dear Friend of Windham Hospital:
The Auxiliary to Windham Community Memorial Hospital is holding its 13th annual "Autumn Gala" on Saturday, November 15, 2008 at the Marriott Ballroom at Adrian's Landing in Hartford. An integral part of the festivities is the recognition of both the Physician and Caregiver of the Year.
We hope every member of the community will consider nominating their physician or other caregiver for one of these prestigious awards. Attached you will find the nomination form along with a list of past honorees.
Criteria for consideration
Currently affiliated with Windham Hospital
Active in the affairs of the Hospital
Participates in community activities
Contributes to the improvement of health
in the community
Exemplifies the best in the Physician/Caregiver-patient
relationship
Completed nominations must be submitted by August 19, 2008 to:
